1. Letter of Intent addressed to the Regional Director through the Schools Division Superintendent
2. Implementation Plan
3. Training Plan for parents/guardians on the following:
a. Curriculum
b. Instruction
c. Assessment
4. Homeschooling Program Calendar
5. Designated Homeschooling Program Coordinator
