Issuance of Certificate of Closure and Clearance to Private Schools

Who may avail of the service?

Private School (Kindergarten/Elementary/Junior High School/Senior High School/SPED) from different divisions of National Capital Region

What are the requirements?

1. Letter of Intent addressed to the Regional Director

2. Certification under Oath by the School Head/Board Resolution stating the following:

a. the reason for closure;

b. that the school facilitated the transfer of learners to the nearby schools with parents’ approval;

c. that the school notified its personnel on the termination of employment due to the closure of the school; and

d. Undertaking that the school shall furnish necessary credentials, clearance, and other scholastic records to learners affected by the closure.

3. Certification issued by the SDO that the school has surrendered the complete school records of all its learners for safekeeping and future reference.

Checklist of requirements for closure

- Issue the necessary transfer credentials (Form 138 and other documents) of all its learners affected by the closure;

- Turn-over the permanent record of students (Form 137) to the Division Office;

- Inform all affected parents as to when the transfer credentials shall be made available to the Division Office for safe keeping; and

- Notify the Department of Labor of its closure as there are employees affected.
